Enter a survey

Click on the My Survey main menu item.  Or mouse-over the My Survey menu item and select the Enter My Survey option.

Display my survey information:

Hover your mouse over the My Survey main menu item, then click the Display or Delete My Survey option.  You will be required to input the email ID and "secret word" that you used when you completed the survey.  Click the Display My Survey button.  This will identify your survey and display it on the screen.

Delete My Survey Information

Hover your mouse over the My Survey main menu item, then click the Display or Delete My Survey option.  You will be required to input the email ID and "secret word" that you used when you completed the survey.  Click the Delete My Survey button.  This will identify your survey and delete it.  You will see a message that your survey was deleted.  If it can't find a match with the email ID and secret word you entered, it will tell you it could not find a matching survey.  If you can't remember your email ID or secret word, send us a message  and I will delete it for you.

Change My Survey Information

Currently, the only way to change your survey submission is to re-enter your survey.  Your prior survey data will be deleted and your new survey information will take its place.

Get a Logon ID

You do not need a logon ID to enter a survey.  However, to run reports and view survey data, you do need a logon ID.  Logon IDs are issued to ward leaders, and permissions granted to view the data for their ward.  Contact us to request a logon ID.
